未成年者のご宿泊につきまして / Parental Consent Form for a Minor staying in our Hotel

ご宿泊者様が未成年者の方（お一人様または未成年者同士）の場合、
親権者様に「未成年者の宿泊に対する同意書」の提出をお願いしております。

同意書は未成年のお客様1名につき1枚のご提出が必要となります。
親権者様がご同意のうえ、事前に必要事項をご記入・ご捺印いただき、
チェックイン時にフロントスタッフまでご提出ください。

チェックイン時に親権者様の同意が確認出来ない場合は、
ご宿泊をお断りさせていただくことがございますのでご了承くださいませ。

なお、ご記入いただきました個人情報は同意書の趣旨に限定して利用させていただきます。

Parental Consent Form for a Minor staying in our Hotel

In the case of a minorr (a person under age 18) staying alone or in a group of minors only, we would request to the parent (guardian) to fill out this
form and submit the sheet to the hotel.
Please fill out the form of all the members.
If we cannot identify the parental consent upon check-in, the hotel staff may contact the parent/guardian for oral consent,
and will request to fill out the form that needs to be submitted by email, fax or post.

The information here will be used only in terms of the agreement for the minor’s stay.
This private information will not be shared without your consent, except when the law allows.
